Abstract and Introduction: Infections as a rare cause of rhabdomyolysis. Legionella pneumophila can cause severe illness. Legionnaire's disease, rhabdomyolysis, and acute kidney injury linked to high morbidity. Mechanism of muscle and kidney injury unclear. Case Presentation: 53-year-old man with Legionella pneumonia, rhabdomyolysis, and acute kidney injury. Treated with fluid resuscitation and azithromycin. Recovery without renal replacement therapy or ventilation. Early recognition and treatment crucial for recovery. Discharged without rhabdomyolysis recurrence. Conclusion: Legionella should be considered as a cause of rhabdomyolysis due to its severe outcomes.
