Abstract and Introduction: Chronic VLUs are challenging to heal. Timing and combination of treatments are crucial. Case report combines NPWTi, biofilm killing solution, hydrosurgical debridement, and STSG. Successful wound healing in 2 months. Reduced time to healing compared to standard care. Introduction: VLUs are a late sequela of chronic venous insufficiency. VLUs are an economic burden. Compression therapy is standard but not always successful. Larger ulcers have longer healing times. STSG is an option for large VLUs. NPWTi shows promising results in wound healing. Case Report: Patient with chronic VLU on anteromedial ankle healed in 2 months. Combination of NPWTi, hydrosurgery, and STSG led to successful healing.
